Responsibilities:

• Conduct in-home evaluations and treatments for outpatient physical therapy/ OT patients.

Requirements:

• Licensed Physical Therapist or Occupational Therapist (PT or OT) in Arizona (or eligible).
• Minimum 1 year of outpatient or home health related experience preferred.
• Reliable transportation for travel within the Phoenix metro area.
• Self-motivated with excellent interpersonal skills.
